2. The authorized owner(s) and the Proposed Insured can complete the application online or print and fill it out by hand using blue or black ink. In some cases the authorized owner and the Proposed Insured may be the same person. Review all of the instructions provided on the form as it is completed.

3. After printing the application, the Proposed Insured and authorized owner(s) are required to sign the form. Make 2 copies to keep and mail the original application to:

   Tip: An application is required for each person coverage is being applied for (or added to).
